Patient Portal Scheduling: If you have already created a Luminello patient portal account and wish to self-schedule or reschedule an appointment (with at least 24-hour notice), view practice documents, or send a secure message, please click the link below.

Please complete all forms before the scheduled appointment, and submit any past psychological testing/evaluations, lab work, EKG reports in the last 90 days, and school documentation, including the last report card and a copy of your child's IEP or 504 plan to the portal.
